Not sure if my friends in the USA are aware of the fact that citizen of other countries (Switzerland, in my example) need to fill out forms like this when dealing with banks and Bitcoin services. Imagine we all had to do this for each country. Of course, we don't. What do you think when you see this form? Yes, me, too: Switzerland is not independent.
