This is called a sucking chest wound. It was caused by a propeller hitting this guys flank. It is sucking air into his chest cavity and collapsing his lung. You need to tape a sheet of sterile plastic over the wound and tape it on 3 sides to make a one way valve so it seals on the inhale.
