You can solve it with (choose your cocktail of): proof of work, captchas, KYC, burning bitcoin, global PageRank scoring, making friends IRL, automated content analysis + discovery algorithms, people purposely viewing the #introductions hashtag, transposition-based web of trust algorithms, etc. Which is good, because it's a real scenario, that we can't really ignore: "I heard about this thing called nostr, maybe I'll try it".
