HTLCs go to fees for the commitment transaction when the HTLC amount is too low. This is incentive compatible, because failing to follow the protocol results in the other side closing the channel, depriving the attacker of fee revenue. All this works.

The only problem here is poorly coded lightning implementations that underestimated fees. That's a fairly easily fixable problem.

The super majority of payments that my Lightning nodes route are under 500 sats, and thus not actually represented with HTLCs. That's dozens of payments a day without problems. Bigger nodes of course do even more traffic.
