One of the curious aspects of the NHS is that while management and ownership systems are reguosrly reinvented, the structure of services has not changed much since 1948.

The 1948 model was GP practises and hospitals. 80 years later, both sectors have consolidated: GP practises have merged, cottage hospitals are almost gone, and hospitals are much fewer and much bigger.

Other types of services are rare. Minor surgery centres, drop-in clinics etc, should proliferate but don't.
