I'm being a smartarse about this of course but let's say we actually had the will to fix it, what would that look like?

I'd put my money on a real commitment to shared learning across functions. Everyone should be able to explain their work in ways that are understandable to anyone else in an organisation, and in doing so they would deepen their own understanding of it.

Together they would develop a shared understanding of how each role contributes to the whole.
