That's a prohibitive cost to prevent abuse of the system. I've seen what the ambulance system looks like in Canada, and it's a fucking nightmare.
Grannies with the same a-fib they've had a million times taking up stretcher space in emerg hallways waiting for beds to clear. One less truck on the road to respond to a 18 y/o kid who got side swiped by a drunk driver, and granny's gonna get a $35 bill in the mail.
EMS is for urgent on-scene >>rapid intervention<< of life threatening complications.
All in saying is, if every ambulance service in america rendered care and transport for free, no one will be showing up for you when you need it and the cost is irrelevant because it means the life or death of yourself or a loved one.
